All new clients are required to complete intake paperwork before their first appointment.
Please begin by completing the New Client Health Intake Form linked below. This form helps me understand your health history, current concerns, goals for care, and any precautions that may affect your session.
Required First Step
This form must be submitted prior to your first session so I can review your information and determine whether corrective bodywork is appropriate for your needs.

Returning Clients
Returning clients may be asked to update paperwork if there has been a major health change, new injury, surgery, pregnancy, medication change, or if it has been a long period of time since your last visit.
